Here’s a DRAFT consent form that site teams can use if they are looking to get permission from youth participants and their families/carers to use content (photos, videos, comments from social media… the sky is the limit!) publicly.
Feel free to let me/us know if you need help changing things around on this, and please use it as a guide and change it to suit your site’s needs. Most important, of course, is that all participants give informed consent to the use of their image/voice in all that we do!
